Register With The Practice
We are always happy to accept new patients who live in Waltham Forest, within our practice catchment area (E11 and E10). To register with the practice, you will need the following documents: Photo ID - passport, utility bill / council bill (dated last 3 months), NHS Medical Card (if already registered with NHS).
All new patients are required to have a new patient health check with the practice nurse / health care assistant to complete registration. This is dedicated appointment for you to discuss your current health conditions and an opportunity for the staff to assess your current health.
Appointments may be booked with the GP once this has been done.
We are participating in a new NHS Initiative to ask patients four additional questions about their lives. Please watch the video below about the questions. We will ask these questions when you first register with our practice but if your answers change at any time, please let us know so that we can support you.
